Shoulder is the most mobile joint of the body. This allows you to lift your arm, rotate your arm and lift your arm over your head. It’s possible to have a greater range of motion with less stability.

How

Shoulder instability

This happens when the humerus head (the upper arm bone) is forced out of the shoulder’s cavity. Usually this happens as a result of a sudden traumatic injury.

Once the shoulder is dislocated, the shoulder is vulnerable to repeat. When the shoulder is loose and slips several times, it’s called a chronic shoulder instability.

The shoulder is made of 3 bones : humerus (upper arm bone), scapula (shoulder blade) and clavicle (collarbone).

This may be partial, which means that the arm’s ball partially comes out from the cavity. This is called a subluxation. This can be complete which means that the arm’s ball comes out completely from the cavity.

Symptoms

Symptoms of chronic shoulder instability are :

  • Pain caused by the shoulder injury

  • Repeated shoulder’s dislocation

  • Repeated instance of the shoulder giving out

  • A persistent sensation of the shoulder that is loose, slipping out of the joint or hanging.

Diagnosis

Specific tests help assess shoulder instability (including general relaxation of ligaments). A doctor may prescribe imaging tests such as X-rays, CT Scan or MRI to confirm the diagnosis and identify other problems.

Treatment

First, chronic shoulder instability treated with nonsurgical options. If these options don’t relieve pain and instability, surgery may be needed.

Nonsurgical treatment

Generally, it often takes several months of nonsurgical treatment before success can be assessed. Nonsurgical treatments includes :

  • Activity modification

  • Non-steroidal anti-inflammatory medication

  • Physical therapy

Surgical treatment

Often, surgery is often required to repair torn or stretched ligaments so that they can maintain the shoulder joint in place.

Bankart lesions (tearing of the front labrum from the cavity) can be repaired surgically using suture anchors to reattach the ligaments to the bone.

Arthroscopy => Soft tissues of the shoulder can be repaired using tiny instruments and small incisions. It’s a procedure that is done the same day or outpatient. Arthroscopy is a minimally invasive surgery. The surgeon examines the inside of the shoulder with a small camera and performs the operation with special instruments.

Open surgery => These are patients who require open surgical intervention. This involves making a wider incision on the shoulder and performing the repair under direct visualization.

Rehabilitation

After surgery, the shoulder can be temporarily immobilized with a sling. When the sling is removed, it’s essential to do ligament rehabilitation exercises. These exercises improve the range of motion of the shoulder and avoid scarring during ligament healing. Thereafter, exercises for strengthening the shoulder will be added in the rehabilitation program.

Meniscal tear is one of the most common knee injuries in athletes. This is caused after a contact injury or a traumatic twist. The meniscus is a wedge-shaped cartilage that provides a cushion in the medial and lateral portion of the knee joint and acts as a « shock absorber ». It’s located in the area of joints contact to prevent the bones rub between them because of the bodyweight. It’s hard and rubbery to help cushion the joints and keep the knees stable.

It’s important to also know that the meniscus helps to nourish the knee by facilitating the diffusion of joint fluid. With this injury, athletes can experience acute pain with a swelling and often a catching or locking sensation.

Diagnose

A meniscus injury can be diagnosed on the basis of the story that the patient provides and a physical examination of the knee.

An orthopedic surgeon my also use other techniques to further diagnose such as magnetic resonance imaging (MRI), which gives a 3-dimensional image of the inside of the knee joint. In some cases, a surgeon can perform an arthroscopic inspection of the articulation, this is a minimally invasive surgical procedure.

Treatment

A small meniscus tear can be treated with rest, ice and anti-inflammatory medications. When a meniscus tear causes a significant loss of movement or catching, the appropriate treatment is often a surgical operation. Depending on the location and type of tear, the treatment may be a simple arthroscopy to remove the torn fragment. Depending on how quickly the inflammation disappears, athletes can return to a full activity after a week or months.

For athletes with a repairable tear, sutures are used to sew the meniscus. In this situation, the knee is braced for 6 weeks. These athletes take at least 3-6 months to back to their activities but maintain the full cushion in their knees.

Prevention

There is not really any way to prevent a meniscus tear apart from a conditioning program to try to prevent an acromioclavicular ligament (ACL) tear and knee instability.

Stats

Women have meniscus tears more often than men and at an earlier age. These tears are often associated with an acromioclavicular ligament injury (ACL)

There are some variables of meniscus tears can be repaired and despite repair, they aren’t always heal. But arthritis can be avoided in the majority of cases when repair is successful. One study shows that 60% of patients who undergo meniscectomy (partial elimination) had some degree of progressive arthritis.

I often meet entrepreneurs who do their own accounting and they say it’s a nightmare. Here are the 2 reasons why I advise them to stop :

  1. Accounting requires a lot of time.

  2. It’s extremely easy to make a mistake. Even certified public accountants make mistakes. The problem when there is an error in your accounting is that all the numbers are wrong. And when all the numbers are wrong, you tax return is false and when your tax return is false, you are in a long-term nightmare.

It’s possible to find a good accountant for around $1 200 a year when you start a company. It’s always better to pay that than to be in a financial nightmare for 5 years because of a mistake in your tax return.

What’s cool is that with WordPress and its free plugins and themes, it’s easy to install and manage a blog / website. There are also other platforms that allow you to do the same thing.

On the other hand, when there is a major problem that exceeds your skills, it can be negative for your company. For example,your blog / website no longer responds. If you don’t know how to do, you can spend 2-3 days trying to understand the problem and it’s been 2-3 days since your audience no longer has access to your blog / website.

At that moment, you need an IT provider. This is not a person you will hire as an employee but it’s a person to whom you can entrust all the technical tasks that you don’t knew how to do.

Yes, it will cost money but ask yourself this question : « Is it better to spend 2-3 says repairing your blog / website is down or pay a IT provider who repairs it in 1 hour ? ».

I advise you to find a IT provider quickly when you start making money with your company before having a major problem. Maybe someone around you knows a good IT provider or maybe you need to find one on Upwork. Test this IT provider with a small task to find out if her is competent or not.

It’s something to delegate but not as fast as the technique. At the beginning, it’s important to do customer support yourself to be in direct contact with the audience to see the problems in services or products, etc. But that doesn’t mean that customer support is the best added value task that the company’s founder can do.

With customer support, you will quickly see that there are often the same questions that are asked. To eliminate much of the customer support is to make a FAQ. A FAQ is the list of questions most frequently asked with a reply.

For other questions that are a little more complex that are are common, you can do a procedure to explain to the people who take care of your customer service how to solve the problem. And you can ask them to inform you when a question is frequently asked to have a permanent feedback. This allow you to stay in touch with reality.

Maybe someone around you knows a good customer support provider or maybe you need to find one on Upwork. Test this customer service provider with a small task to find out if her is competent or not.

Social media

Since you have a blog or a website, you have an audience. This audience sends you messages when you create content. This is the same principle as for customer support, it’s important at the beginning that you answer the messages yourself to discover your audience. You will know the fears and the hopes of your audience. It’s important that you learn how to communicate with your audience.

To find out what things to delegate, you need to be aware of seeing where you bring the most added value to your company as a founder, it’s about innovation, strategy and setting up processes. This allows you to work on your company and not work in your company.

It’s important to know the difference between working on your company and working in your company (being a slave to your company).

You will find providers from several different countries. Always test a provider with a small task that doesn’t have a huge impact on your company to make the selection.

Muscle Memory

I read a Nerd Fitness article  and there is good stuff.

Muscle memory is an action execute with a lot of repetitions physically or mentally. The body will learn to execute this action more effectively using less brain power.

Your muscles don’t have brain but rather your brain learn to create a procedure list to execute tasks as quickly and efficiently as possible. The majority of the time you do these tasks in mode « procedure » of your brain and this become automatic. It’s like cache memory in your computer or smartphone.

When you play at the first time to a video game, you must learn how the controls work (and you’re really bad). You must learn to :

  • Look left and right

  • Use gun/sword

  • Jump and rolling

  • Move, avoid enemies

  • Attack, defend, cast spell

After several hours of gaming, your muscles begin to memorize. After several days or weeks, you do these things effortlessy.

  • Defeat a boss with combos, rolling, attacking, defending and spell casting.

Muscle memory isn’t only for video games. This helps for several things : driving, cut vegetables, play a musical instrument, etc.

When we decide to be healthy in the real life, this concept of « automatic response » is an important element.

Unfortunately, this can have bad effects :

  • If you spend several months to learn badly to do squat, it’s very difficult to stop this bad habit and relearn the squat in the right way.

  • If you learn to play a song in a bad way (or another activity), you learn to your body « it’s the good way to do it », even if it’s the bad way.

  • When you are busy at school/work and take candy and soda to manage stress, you brain will be train to say : »it’s the most effective way to be happy ».

We really talk about to build habit. Muscle memory can help us to understand how and why we failed in the past and how to do to change it

We’re creature of habits. The things we do everyday like the way we walk, the way we talk, the way we sleep (or not), things that make us happy or sad are the result of hundred and hundred of repetitions of these activites.

When we try to create new habits, it’s a lot, a lot of work.

For exemple, rather than going to McDonald’s, we must remember of :

  1. Go to the supermarket

  2. Take vegetables and chicken

  3. Go home and prepare vegetables

  4. Cook chicken

  5. Prepare the table

  6. Eat

If you don’t have these habits rooted in your brain, just read this list can be exhausting.

Look this exemple : rather than stay a home to watch the TV, we decided to learn squat :

  • Get under the barbell

  • Tighten the shoulder blades and place the bar correctly on the trapezoids.

  • Inhale deeply and go down

  • Knees lined up with toes

  • Explode back up with proper form

You saw these 2 cases, there is a lot of change !

Automatics things like going to McDonal’s or watch TV, come easy. We’ve always done them and thex require little effort. It’s like a second nature. The second part (new habits) require a lot of amount of effort (don’t worry, you can do it). I tell you, there can have barriers or closed doors the first few times but you must not give up.

After all these repetitions to have healthy habits, I can confirm that train and eat heatlthy is a second nature for me !

How to build

In the Malcolm Gladwell’s book « Outliers » , he talk about the concept to master. In his research, he found the the most experts spend around 10 0000 hours to master their professions.

The most important is to not to spend 10 000 hours to practice. It’s to spend 10 000 hours to do the good practice.

Play the same song on piano in the wrong way during 10 000 hours will only allow you to play with eyes closed this song in the wrong way.

Work actively on your technique with a coach or a teacher to focus on imporving all aspects of playing the piano.

To build this muscle memory in our life, we don’t need to spend 10 000 hours of practice living healthy but we need to practice. Without do regulary a healthy activity, you will never feel your new healthy activity like a second nature.

You need to :

  • Take one or two new habits you want to create. No more !

  • Make things easy to do this activity. Minimize difficulties between you and this new life activity that you want build. Find a way to complete this activity using the minimum of willpower.

Exemple :

  1. Take one or two new memory : Rather than to completly change your training program to stop to use machine and start to use barbell/dumbbell, change just one exercice. Instead to do Leg Press, do squat. After 3 weeks when you feel good with squat, you can change another exercice.

  2. Minimize difficulties for the new activity : Do you want run every morning ? Sleep with your running clothes, put your alarm clock across the bedroom and put your running shoes next to the alarm clock. You are half ready, right ?

  3. Remove the need to remember what you need to do : I rename the alarm of my smartphone « Meditation ». When I wake up, my smartphone tell me to do my meditation.

  4. Eliminate the need to use the willpower : I spoke of willpower. We all know we need to eat healthy and do more exercice but we don’t do it. Then when we try to rewrite our DNA and our second nature, we must be strategic.

    If you are always tired after school/work and you miss your training, maybe is better to do your training in the morning before to go at school/work ?

    If you struggle with unhealthy foods during stress moments, do a « purge » and throw out unhealthy foods from your house. It’s harder to eat junk foods when you must go out to buy one.

    If you struggle to stop to pick foods from fast foods before you go home, change streets. Choice different streets to go home, streets without fast-foods.

The essence of all that has above is the practice. You train your brain/body as an integer to build healthy automatic habits.

Momentum is a powerful thing : If you can’t do all these healthy habits when you life become crazy. Take one or two healthy habits you can do and continue to practice. By this way, you don’t need to start back at square when your life will return to normal.
