Definition

Articular cartilage is difficult to understand because there are 3 types of cartilages in the body : articular of hyaline cartilage (covers joint surfaces), fibrocartilage (knee meniscus, vertebral disk) and elastic cartilage (outer ear). These cartilage’s types differ in their structure, elasticity and strength.

Articular cartilage is a complex element, it’s a living tissue that is on the joint’s surface. The function is to provide a low friction surface to allow the joint to withstand weight loads through the range of motion needed to perform activity of daily living. To put it simply, articular cartilage is a very thin shock absorber. It’s built in 5 distinct layers and each layer has a structural and biochemical difference.

Articular cartilage injury may be due to trauma or progressive degeneration (wear and tear). This can be mechanical destruction, a direct blow or other trauma. The healing of articular cartilage cells depends on the severity of the damage and the location of the lesion. Articular cartilage has no direct blood supply so it has very little ability to repair itself. It the lesion penetrates the bone under the cartilage, the bone provides blood in the area which improves the chances of healing.

Mechanical degeneration (wear and tear) of articular cartilage occurs with progressive loss of normal cartilage structure and function. This loss begins with the softening of the cartilage, then progresses to fragmentation. As the loss of articular cartilage lining continue, the underlying bone no longer has any protections against normal wear and tear of daily life and begins to get damaged leading to osteoarthritis.

In many cases, a patient experiences knee swelling and vague pain. At this stage, continuous physical activity isn’t possible. If a loose body is present, words such as « locking » or « catching » might be used to explain the problem. With wear and tear , the patient often experiences stiffness, decreased range of motion, joint pain and/or swelling.

Diagnostic

The physician examines the knee to look for a decrease in range of motion, pain along the joint line, swelling, fluid on the knee, abnormal alignment of the joint’s bones, and ligament or meniscal injury.

Cartilage lesions are difficult to diagnose and it’s possible that the use of magnetic resonance imaging (MRI) or arthroscopy may be necessary. Plain X- rays don’t usually diagnose articular cartilage problems but they used to rule out other abnormalities.

Treatment

Articular cartilage injury that doesn’t penetrate the bone doesn’t repair itself. A lesion that penetrates the bone can heal but the type of cartilage created is structurally unorganized and doesn’t work as well as the original cartilage.

Lesion less than 2 cm have the best prognosis and the best treatment options. These options are arthroscopic surgery using techniques to remove damaged cartilage and increase blood flow from the underlying bone (drilling, pick procedure or microfracture ).

For smaller lesion of articular cartilage surgery is not required.

For larger lesion, it’s necessary to transplant the articular cartilage from another area of the body. Talk to your doctor or specialist to have more information about the decision to have a surgical operation.

For patients with osteoarthritis, non-surgical treatment consists of physical therapy, lifestyle modification (for example reducing activity), bracing, supportive devices, oral and injection drugs (like non-steroidal inflammatory drugs, cartilage protective drugs) and medical management.

Surgical options depend on the severity of osteoarthritis and may provide a reduction in symptoms that are usually short-lived. Total osteoarthritis may relieve the symptom of advanced osteoarthritis but this usually requires a change in the lifestyle and/or level of activity of the patient.

Statistics

Based on published studies, the overall prevalence of articular cartilage injury in the knee is 36% among all athlete and 59% among asymptomatic basketball players and runners.

How To Reduce Stress (Part 1)

I watched an Olivier Roland’s video  and I learned good stuff.

I am doing a synthesis of the Dale Carnegie’s book “How to stop worrying and start living”  and other sources on internet for a blueprint to manages stress in 4 steps.

Stress is about change, it means adapting to the environment of a changing structure. Stress is a feeling triggered by the interpretation of an event. It’s a reaction related to the constraints and the pressure undergone in its environment. Stress can be interpreted as something positive, whether it’s considered a challenge or as something negative, if it’s considered a harmful event.

The truth is that stress is a mistake of reasoning in relation to the attitude or behavior we adopt. This is an answer that isn’t adequate. It’s an alert for mental dysfunction as the body gives us an alert for physical pain. Fortunately, people can learn to control stress in 4 steps : Take a step back and analyze what triggered the stress ; anticipate and visualize ; organize yourself and communicate and turn away or adapt and accept.

1st step – Take a step back and analyze

It’s necessary to step back and analyze what triggered the stress. Because before trying to solve that, it’s important to know what caused it. By analyzing what caused the stress, we can avoid certain consequences.

To do that, here’s what you can do :

  • Make a list of sources of annoyance that you have in your family, friends, school, work, transportation, etc. Once you have found them, classify them from the most impacting to the least impacting. You can also do a 20/80 analysis, that means identify the 20% of the sources that bring you 80% of your stress.

  • Ask yourself these questions : « What is the problem ? What is the cause of this stress in these sources ? What are the most serious consequences that I suffer of my experience ? »

  • Understand the emotion that created in these moments. Then check if it’s a real fear or rather a theoretical concern that doesn’t have much to happen in your life.

If a situation is truly unavoidable, you need to prepare yourself to accept it. We must give up the impossible because it’s extremely stressful to have no way to change a situation. To avoid this, you need to anticipate situations that may lead you into an impasse. Once these situations identified, you must try to really avoid them or change the course of things from the start.

It’s impossible to keep control of all things in your life. It’s useless to set inaccessible goals.

2nd step – anticipate and visualize

The best way to handle stress is to anticipate. Based on the list you made in the first step (source of stress), imagine solutions and possible answers. Ask yourself these questions : « Why did you agree to be in this situation ? ». If what you do make sense, it limits the stress.

Decide how you want to live a situation. An event that happens to you can have not effect on you until you choose words to qualify it. If you say that an event is happy, boring, sad or extraordinary, it can become so. By qualifying the event, you modify the perception that you have of this event.

The way you see the event determines your body’s reaction. So take a moment to think before judging and classyfying a situation as negative.

Take Olivier Roland’s example. When Olivier Roland began to meditate, his neighbor’s dog was barking. At first it disturbed Olivier to focus on his breathing, it was stressful and annoying. After a while, Olivier realized that it wasn’t the barking dog that was stressful, it was Olivier’s interpretation of the situation. From the moment that Olivier decided that the barking dog was something neutral or positive, he began to appreciate when the dog barks. It’s a sign of life.

Imagination can be used to help fight stress. Get in shape before a stressful event. You can create a positive state of mind when an event is negative. Think of something positive like music, a holiday, projects in progress. Visualize a happy moment, a happy memory.

Install Tableau Desktop and Tableau Public

I have just enrolled in a Data Science course on Udemy  and I learned good stuff.

Today, I’ll show you how to install Tableau.

Use your web browser to go to https://www.tableau.com/ . On the homepage, there are several options of packages to be installed.

For the course that I do, there is the choice between 2 versions : Tableau Desktop and Tableau Public.

Tableau Public charged and of course, there is a trial version of 14 days. Tableau Public is free and is perfectly usable for this course because there are the same features as Tableau Desktop.

Install Tableau Desktop

Click on the « Try Now » button on the top right

Here there are 3 options : Tableau Desktop, Tableau Online and Tableau Server. What interests us is Tableau Desktop and for that we need to give an email. Once you gave your email, the installation file downloaded automatically.

Don’t worry, the download contains the correct version of the file. This will be a « xzy.EXE » file for Windows and a « xyz.DMG » file for Mac.

If this isn’t the right version that has been downloaded, you can always do this manually with these links.

As soon as the download is complete, you can watch tutorial videos in the « Get Started » area. It’s simple and intuitive.

Install Tableau Public

To install Tableau Public, it’s a little different because you have to go to another website. Use your web browser to go to https://public.tableau.com/s/.

Like for Tableau Desktop, you need to give an email to install the installation file automatically.

The majority of people who use Tableau Public are journalists or public organizations that do not have confidential data because Tableau Public data is on public servers. And as you know, data of a public server is freely accessible to everyone on internet.

Tableau Desktop and Tableau Public have the same structure, features and visualization layout.

But there are 3 differences :

  • With Tableau Public, you can’t extract data because your data is on a public server.

  • With Tableau Public, you need to make screenshots (printscreen) of your charts because you can’t export them.

  • With Tableau Desktop, you can connect to more file type and you can export your charts.

How To Plagiarize Others Smartly

I watched an Olivier Roland’s video  and there is good stuff.

Plagiarize others smartly without copy what they do but have an efficient inspiration. It’s possible copying something people don’t think about.

Olivier Roland talked with a youtuber and this youtuber told him he has completely plagiarized Casey Neistat. Casey Neistat  is a famous youtubers with more than 80 millions views. The truth is this guy copied the video’s structure of Casey Neistat. The thing is copy the sturcture of something that works and not the content.

Star Wars

You know Star Wars of George Lucas, these films are famous but George Lucas copied a structure. Joseph Campbell wrote a book called « The hero with a thousand faces ».

In this book, Joseph Campbell analyzed a dozen of myths around the world. Old myths we still hear today. He did it for several reasons. First, for him if there myths with thousands of years are still told today, there is a natural selection for myths. Only myths interesting to the human mind have survived. Second, if we analyze myths from Africa, Europe, oceania, asia, america, etc, we can find the common point they have. Joseph Campbell found what he call the « monomyth ». It’s a narrative structure in stories that talk deeply to human mind in a universal way.

When George Lucas discovered the Joseph Campbell’s work, he had already begun to write Star Wars but it was a revelation and he rewrote a big part of the scenario following exactly the Joseph Campbell’s sturcture. There is a lot of movie and books that use this structure like Matrix. It’s funny to compare the structure of Star Wars and Matrix with the monomyth’s structure.

It’s always smart to copy structures that work. When you see something that work don’t look the content as 90 % of people do, try to analyze the structure. In which order the content is presented, what rhythm, what type of interruption, etc.

Olivier Roland did improvisational theater for 3 years. The concept is to ask to spectators a subject and create a story with this subject. It’s not easy but every story has the same structure so just take the subject and tell it with this structure. Improvise the content without improvising the structure, it’s cool.

This is the monomyth of Joseph Campbell :

  1. A hero receive a call to adventure. In Star Wars episode 4, it’s 2 droids that have a message from the princess Leia to Luke.

  2. The hero refuses the call to adventure by this way we can be more connected with him/her. We appreciate him/her because he/she like everybody. He/she is not really courageous. A hero is someone who become a hero. In Star Wars episode 4, Luke doesn’t really refuse. It’s his uncle and aunt who tell him to stay at the farm.

  3. A trigger will encourage the hero to accept the call of adventure. For Luke, it’s his family that gets killed.

  4. During the trip, there is a lot of problems, incidents and the hero will meet people who help him/her. Typically there is as archetype the old sage, the princess, etc. There are also enemies who will put the hero to the test.

  5. At the end, the great confrontation with the wicked.

Here is the structure of all classic stories. Braveheart has the same structure. You can watch a youtuber and try to understand the video’s structure. It isn’t easy to do reverse engineering because we aren’t used to doing it. We don’t think about it but you have to know that the structure make at least half of the success of something.

We can copy at 100 % a structure. It can make at least half of the success without anyone noticing it and without ethical problems because a structure is a method. It’s cool, right ?
